TASK 1
In this task you are expected to visit any industry of your choice and conduct an incident
investigation. Based on your observation, you are to prepare a detailed Health and safety
Report. The report should consist of approximately 1500 words.
The Report Should Focus on identifying the role of human factors in the occurrence and
consequence of the chosen accident. It should contain a brief introduction describing the
background information of the case, identifying the specific pre-accident strategy and explain
the features involved in each strategy with relevance to the case study, recommend postaccident (reactive) strategies fitting intended for the case, conclusion and supported
references.
You are required to refer from wide range of literature review materials and sources to write
this report. Please refer to the Guidelines for preparing the report.
The report should be preferably submitted in the following format:
1. Introduction- general introduction on human factors about the incident

9 All symbols and abbreviations must be defined, and used only when absolutely
necessary.
10 The number of words must be shown at the end of the work submitted. The title page,
acknowledgements page and contents page will not count; neither will the list of
reference at the end of the work. Footnotes will be counted; in any case their use is
discouraged. If your work requires a larger proportion of non-verbal forms of
presentation, it can be agreed with the module tutor how these will be calculated.
11 Referencing
Referencing is a system that allows you to acknowledge the contributions of others in
your writing. Whenever you use ANY words, ideas or information from ANY source in
your assignments, you must reference those sources. This means that if you use the
exact words of an author, if you paraphrase their words or if you summarise their ideas,
